Eligibility / Target Population:

Patients that are admitted to Ontario Health atHome Brampton can attend a nursing clinic
•not open to general public •must be assessed and referred to the clinic by Ontario Health atHome Central West staff •visit to clinic by appointment only, no walk-ins accepted •patients must be covered by OHIP
