Service Description: Provides timely, short-term, community-based mental health and addiction support to individuals experiencing heightened vulnerability. The program helps individuals stabilize, work on goals and connect to additional appropriate supports In-STED is designed to reduce repeat and non-essential visits to emergency departments by addressing needs earlier and more holistically. The team works collaboratively with individuals, families, and community partners to provide short-term case management supports, service navigation, and warm linkages to ongoing community-based services Referrals to the program are received from Trillium Health Partners, supporting seamless transitions from hospital to community care and promoting continuity, dignity, and recovery-focused outcomes

SHIP Mission: To optimize quality of life for individuals with mental illness through housing and community-based services SHIP provides a variety of community based mental health and supportive housing services for men and women who have an identified psychiatric illness, enabling them to live independently in the community in an atmosphere of support, co-operation, understanding and affordability
