Service Description:
|
CHO provides shared accommodation and supportive programming for individuals living with serious mental illness who may benefit from a 24-hour supportive care environment within the community
The program is grounded in a person-centred and recovery-oriented approach, recognizing that each individual’s needs, strengths, and goals are unique and may change over time. Clients are supported as active participants in their own goal setting and care planning
The service approach includes:
- community mental health counselling
- nursing support
- financial literacy counselling
- coordination and brokerage to other health and community services
CHO Oliver is located in Caledon, Ontario, and CHO Thomas is located in Etobicoke, Ontario

Eligibility / Target Population:
|
Adult residents of Ontario with serious mental illness * Require housing and support services to live as independently as possible * Includes those that require assistance with ADLs, some level of 24-hour support * Priority can be given to those who are also experiencing/at risk of homelessness, transitioning from hospital
