Service Description:
|
SHIP has a Central Intake and Access department which serves as a single point of access for all applications and information.
Intake and Assessment team provides:
- single point of access for applications and system navigation
- intake and assessment services
- referral triage for mental health, addiction and specialty services
- centralized waitlist management for housing and specialized programs
- designated Intake and assessment counsellor for applicants on waitlists, providing guidance on eligibility criteria, service options, and next steps
- individualized support for individuals and families to identify the most appropriate services based on needs and circumstances
- timely responses to inquiries from applicants, families, and service partners
- up-to-date system knowledge of community services, resources, and opportunities
