Service Description:
|
The ACE team helps patients remain independent after a hospital stay. Using a patient and family-centred approach, seniors' experts create comprehensive geriatric care plans to support each patient's mobility and function while they are in the hospital
All ACE team members have specialized training in supporting the needs of geriatric patient. The team includes: physicians, geriatricians, nurse practitioners, registered nurses, registered practical nurses and health care aides, pharmacists, registered dietitians, speech-language pathologists, social workers, discharge planners, physiotherapists and physiotherapy assistants, occupational therapists and occupational therapy assistants
