Service Description:
|
Supports for older adults, and adults with disabilities by providing personal care and homemaking services at 7 sites in Mississauga and 2 sites in Brampton * helps people remain independent in their own homes for as long as possible * care coordination and referral to other community services
Supports for Daily Living (SDL) - Program assists older adults with the essential activities of daily living, which include personal care, medication assistance (receive medication reminders either via phone or in-person), safety assurance program, laundry, purchasing grocery items, light meal preparation, and light homemaking * also servicing clients in the neighborhoods around those sites
Supportive Housing Program - For people who require daily personal support and essential homemaking to live independently * available in designated residential buildings and also in the neighborhoods around those buildings (depending on individual need) * personal support is provided on-site and is available up to 24 hours a day, depending on individual need
Other services include:- care coordination
- foot care services available at some locations (diabetic foot care and health education)
- medication management (partnered with Resident Care Pharmacy)
- provides programs, workshops, and social activities that help seniors build friendships, learn new skills, and stay engaged in their community (workshops, clubs, virtual visitors, tech tutors, village cafe and shop in Brampton)
