Do you know a Hero?
Every day, ordinary people go to extraordinary lengths to improve the lives of those limited by age, illness and disability.
The Central West Community Care Access Centre (CCAC) would like to celebrate these special people through our Heroes in the Home Caregiver Recognition Awards.
Heroes in the Home recognizes caregivers both paid and unpaid. You can nominate as many caregiver heroes as you like — family members, friends, co-workers, healthcare professionals and others. And you do not need to be the recipient of their care to nominate them!
Last year we celebrated over 110 local Heroes in the Home. We would love to celebrate your Hero at our celebrations this fall. You can nominate a hero by completing a nomination form and returning it to us in the following ways:

2. You can call us and nominate a hero at: 905-796-0040 ext. 7756
Nominees will receive a certificate and will be honoured during one of our award ceremonies.
Nominations are open until September 28th, 2015.
